If working 20 hours or more per week, proof of earned income which will include either:
- Income Verification Form (see attached), signed and completed IN FULL by your employer, OR
- Your last six (6) weeks of current consecutive pay stubs, i.e. Six (6) pay stubs if paid weekly, three (3) pay stubs if paid biweekly, four (4) pay stubs if paid semimonthly, or two (2) pay stubs if paid monthly, OR
- For self-employed or paid in cash a copy of last year’s income tax return, with Schedule C and/or a business ledger.
If enrolled in school, bring a copy of the school schedule showing days and times of class.
If you receive any Social Security benefits, bring your award letter or printout from SS. If disabled, provide a doctor’s note.

If child support is not paid through Child Support Enforcement, submit a notarized letter from the absent parent with name, address, and phone number stating the amount paid and how often it is paid.
Notice of Case Action for TANF/Cash assistance.
Proof of other household income earned or unearned.
Birth certificates for all children living in the household.
Social Security card for all family members (optional).
Current picture ID for all parents in the household.
Proof of Broward County Residence, (i.e. current utility bill).
If separated, notarized letter stating date of separation, spouse’s name, address, and phone number. If spouse’s whereabouts are unknown, provide a written notarized letter stating that the whereabouts are unknown; letter must be dated and signed.
Legal custody papers for non-biological children (included in family size).
